All insurance matters relating to the club need to be emailed to Liz Hofierka liz@waverleyoldboys.com

All registered players with Eastern Suburbs Football Association are covered by an Accident Support Program issued by QBE Insurance.

This program is designed  to supplement loss of income and non-Medicare expenses and cover is provided for injuries caused by active participation in officially organised games, official practice and training sessions, and official functions.

At the beginning of the season all registered players were asked to sign the attached Insurance Form document which outlined the details of the cover.

I advise all players and officials to read the attached document and take note of the following points:

·         Non-Medicare  Medical Expenses - Reimbursement up to 85% of non-Medicare medical expenses (net of any recoveries from private health insurance) up to a limit of $5,000.

Claimable expenses are

o   Physiotherapy/Chiropractic/Osteopathy/Naturopathy/Massage/Acupuncture (all following Doctor referral).

o   Ambulance, Dental, Private Hospital (accommodation/theatre fees/prosthesis) and Orthotics prescribed by a surgeon following surgery. Claims for pre-operative care (physio/chiro etc.) are limited to $350 per injury.

o   An Excess of $50 applies to those not privately insured.

o   Expenses incurred more than 1 year after injury are not covered.

 

·         Loss of Income: Temporary Total Disablement of $200 per week (or 85% of average weekly wage whichever is the less) payable for a period of fifty-two (52) weeks — Excess seven (7) days. Medical Certificates certifying ongoing disability is required every 14 days to enable Loss of income claims to continue. PLEASE NOTE: This is NOT available for STUDENTS

** IT IS IMPORTANT THAT YOUR MEMBERS KNOW THAT THEY NEED TO BE REFFERED TO Physiotherapy/Chiropractic/Osteopathy/Naturopathy/Massage/Acupuncture BY A DOCTOR. THEY CAN  NOT CLAIM WITHOUT A REFFERAL

Method to Claiming: (Please read the Sports Injury Claim Procedure in the attached Insurance Form Document)

1.    Fill in the attached Insurance Claim Form

2.    Get a Club Official to sign the form NOT your RTO / COACH

3.    Bring the form into the ESFA Office (1371 Botany Rd, Botany) or get your club to email the form to info@esfa.com.au

4.    ESFA will verify the player and sign the form

5.    It is then up to the player/club NOT ESFA to send the form onto:

GPO Box 4108, Sydney 2001.

THIS FORM MUST BE COMPLETED WITHIN 60 DAYS OF INJURY
